only available for booking GP appointments. We will review,
in due course, whether to open this up to include nurse
appointments. In order to use this service, you will need to
be issued a user name and password.
If you are interested, please ask at reception or email
email@example.com and we will be pleased to
issue you with a username and password.
Please note - in the case of abuse of this appointment
booking service your log-in details will be revoked,
preventing any further use of the service.

IMPORTANT CHANGES TO HOW WE
HANDLE YOUR PERSONAL DATA
We will soon be required to supply your personal
and confidential medical Information to the Health
and Social Care Information Centre (HSCIC). This
information will be taken from the practice in a form
that can identify you. The Practice has NO choice
but to allow the HSCIC to extract this information.
However, individual patients can instruct their
Practice to stop the transfer of their data.

STATEMENT OF PURPOSE
This Statement of Purpose details the Practice aims and
objectives in providing a General Medical Service to our
patient population >
There is a lot of whooping cough around at the moment
and babies who are too young to start their vaccinations
are at greatest risk.
Whooping cough is a serious disease that can lead to
pneumonia and permanent brain damage. Many babies with
whooping cough will be admitted to hospital and they are
at risk of dying from the disease.
Deaths from whooping cough are rare in the UK but more
babies have already died this year than in recent years.
Expectant mothers can help protect their babies by
getting themselves vaccinated against whooping cough
from week 28 of their pregnancy.
